We thank you for your interest in GLA Before and After School Care. Here are the items needed to start the application process.

• 1) Pay Registration and Administration Fee of $50
• 2) Please give daycare a CURRENT PHOTO of the child
• 3) Complete the Registration Form – hand in at daycare.
• 4) Complete the Emergency Consent Form
• 5) Parent Handbook – Sign Read Form.
